B.C. to launch ‘circle of care’ for neurodiverse kids but parents have many questions

VANCOUVER — British Columbia is set to launch one-stop support centres for children with issues like ADHD, autism and Down syndrome, but parents say promises of a no-wait system that won’t require assessments or a diagnosis seem unrealistic due to a shortage of health-care professionals.

The plan is to open 40 so-called family connections centres, or hubs, across the province.

Four are slated to provide services under a pilot program from next year — three in northwestern B.C., and another in the central Okanagan.

The province has given community agencies until July to tender for initial implementation of the program, and contracts are expected to be awarded in the fall. More proposals will be sought for the remaining centres that are to begin operating by 2024 to support neurodiverse kids and youth up to age 19.

Families of autistic children will then no longer get the current funding of up to $22,000 a year until age six, and $6,000 annually to age 18. Instead, they will be provided services through the new system.

Julia Boyle, executive director of Autism BC, said the province has not provided information about staffing and protocols for the “complex transition” to a new system that could have parents of thousands of neurodiverse kids “knocking on the doors of these hubs.”

There is not enough time for consultations with advocacy groups before the pilot program starts, and to fully shift from individualized funding for autistic kids to the new hubs is another major change in two years, Boyle said.

“It makes more and more sense to have a phased rollout,” she said. “That’s ultimately what I’m pushing for.”

Government “spin” over autism funding has created divisions about inequity because many parents of children with other needs have been paying out of pocket, Boyle said. That has led some to accuse the autism community of being “selfish,” she said.

“It’s on the government to come up with solutions,” Boyle said, suggesting funding could have been provided more equitably to all families in need, instead of being taken away from those who had fought to get it.

The first four hubs will be in Prince Rupert, Terrace, Smithers, and Kelowna. But it’s unlikely many parents of autistic kids in those communitieswould choose to immediately give up funding and switch to the hubs, Boyle said, since participation in the pilot program will be optional.

That could hinder evaluation of the hubs before the program is launched provincewide, she said.

Mitzi Dean, minister of children and family development, said her ministry will be “observing where families are able to make that transition and what it is that’s working for that transition.”

Dean said the goal of the new system is to give families access to a team of service providers from speech and occupational therapists to behavioural interventionists and replace a patchwork of programs that are “not working for far too many families.”

“Each family will have a primary member of a team that they will connect with, and they’ll work together to design the circle of care to wrap around their children and youth with support needs,” she said.

Dean did not directly respond to a question on whether psychologists and psychiatrists would be part of these teams but said mental health support and professionals will be available without a referral.

Parents can access the centres when they start noticing their child is not meeting certain milestones, she said.

“The problem at the moment is that services are locked behind a diagnosis,” she said. “And that limits access to services at the time that a parent or family might recognize that there might be a need or an issue. Then that causes that bottleneck as well. Then, even depending on what diagnosis you get, you still might not get services, or you become a case manager for your own child.”

Dean said parents are “exhausted” trying to find and co-ordinate support on their own and will no longer need to make that effort under the new system, which aims to provide disability and support services to about 28 per cent more children earlier in their development.

As for staffing, she said the province is working with service providers to bring multidisciplinary teams together.

“By building a system and having local community agencies be able to employ a range of health professionals and also subcontract with other people who are able to provide services in the community, that’s how we’re going to be able to deliver this system and work in partnership with community agencies.”

Candice Murray, a registered psychologist and clinical instructor at the University of British Columbia, questioned how a child with attention deficit hyperactivity disorder, which is her specialty, would get help at a hub without having had an assessment, which can be time consuming and complicated.

Behaviour, medication, sleep and eating patterns, parenting and triggers are among the many factors that must be considered during an assessment, said Murray, former head of the ADHD clinic at BC Children’s Hospital. Most effective intervention strategies were based on such an assessment, she said.

“Who’s going to be working in the hubs? The child and youth mental health teams don’t take kids with ADHD,” Murray said of the service currently provided by the Children’s Ministry.

“I don’t know if they would be involved in hubs but they don’t have any experience or training, so that’s another concern I have. Where are all these people going to come from, and who’s going to train them?”

ADHD is not officially recognized as a disability, so children often do not get the support they need at school, even after their parents have spent thousands of dollars on assessments through the private system, she said.

“I worry that this is a watered-down version of an appropriate assessment,” she said of the hubs.

Children with ADHD who don’t get help can end up with disruptive behavioural problems, Murray said. Some later suffer silently until depression and anxiety set in, so their needs should be adequately met in the new system, she added.

Jaymie Nelson, whose two children, Emerson, 12, and eight-year-old Amelia, both have ADHD, said it’s hard to believe there won’t be any wait lists at the hubs “because there are wait lists for everything, even paying privately there’s a wait list.”

Nelson said her children have a rapport with their occupational therapist and speech-language pathologist, and she worries about losing access to these professionals under the hub system.

She attended a town hall last fall after the government announced the hubs and said she felt hopeful that children and youth with ADHD could finally get some support, which isn’t provided at most schools because many teachers are not trained in basic knowledge about the condition.

“It was not clear on details,” she said. “There was really nothing ironed out. Initially, it was like, hey guys, this is awesome. But really it is not. There are so many concerns.”

Fire at building under construction in north-end Halifax quickly extinguished – CBC.ca

Witnesses described hearing a loud blast and seeing large plumes of black smoke on Tuesday morning as a building under construction in Halifax’s north end caught fire.

A message on Halifax’s alert system said the fire was at a building in the area of Robie and St Albans streets. About an hour later, the municipality said the fire was out.

The alert warned people who live on the peninsula to close their windows due to smoke from the fire possibly being toxic.

“Residents and businesses near the fire should still keep windows closed and air exchangers turned off until air quality conditions improve in the coming hours as a precaution,” the municipality said on X, formerly known as Twitter.

Halifax Regional Police were asking people to avoid the area during what is normally a time of heavy morning traffic.

“It shook the whole house,” Shaw said in an interview at the scene. “So I came outside … and saw this incredible building here on fire. Big black smoke. Lots of sparks and stuff going.”

Mike Clark was working on the building adjacent to the one that caught fire. He said he was on the 30th floor when the roof of the other building caught fire and propane tanks blew up.

He said construction crews were then evacuated from the building.

“The elevator was shut down and everyone went down the stairs and out the door,” he said. “Everybody on each floor has a horn to check if anybody was left in the building. Sounded the horn and walked down. It was very organized.”

‘Perfect storm’: Quebec farmer protests continue

Quebec farmers are continuing a series of protests that have brought slow rolling tractors to communities across the province’s agricultural regions.

The president of Quebec’s farmers union Martin Caron says producers are struggling with higher interest rates, growing paperwork and fees on plastic products, like containers of seeds, fertilizer and pesticides.

His organization is asking the current Coalition Avenir Quebec government to ensure farmers can get loans with interest rates of three per cent.

A spokesperson for Quebec’s agriculture minister says farmers can get emergency financial aid through a new program and that the government is consulting with the farmers union about reducing paperwork.

Thirty-four researchers compared notes on herds in Alberta and British Columbia based on a study in Ecological Applications and found between 1991 and 2023, the caribou population dropped by half.

However, over the last few years the numbers have begun to slowly rise, as it’s estimated there are now more than 1500 caribou than there were had not restoration effort been made.

Second World War hangar in Edmonton burns in fire

An aircraft hangar built during the Second World War at Edmonton’s former municipal airport has been destroyed by fire.

A spokesman for the City of Edmonton says in an email firefighters were called to Hangar 11 just before 7 p.m. Monday.

The city’s email says 11 fire crews were dispatched to the scene to deal with the heavy smoke and flames and the wooden building later collapsed.

We're still stockpiling reusable bags. Big grocers have adopted solutions, but experts have concerns – CBC News

Canada’s plastic bag ban has had an unintended consequence: a proliferation of reusable bags piling up in basements, closets and, eventually, landfills.

“They’re everywhere,” said environmental researcher Tony Walker. “We’re drowning in them, and we shouldn’t be.”

To combat the problem, several of Canada’s big grocers have introduced solutions. Last week, Walmart launched a free national recycling pilot program for the retailer’s reusable blue bags. Competitors Sobeys and chains owned by Loblaw Companies Ltd. use recyclable paper bags for grocery delivery.

But some environmental experts argue that paper bags are also problematic and that the best solutions are those that help customers actually reuse their reusable bags.

“We just can’t keep giving [them] out,” said Walker, a professor at Dalhousie University’s School for Resource and Environmental Studies in Halifax. “We’re only meant to have a few of them, and we’re meant to use them until they fall apart.”

In late 2022, the federal government rolled out a ban on the manufacture, import and sale of several single-use plastics, including checkout bags. The regulations are being contested in court, but in the meantime, they remain in effect.

The regulations have made single-use shopping bags scarce in Canada, but they’ve also led to the proliferation of reusable bags, especially for grocery delivery.

“It just creates more waste, which is what we’re trying to avoid in the first place,” Walmart customer Udi Sela said in a CBC News interview in late 2022.

At the time, Sela, who lives in Maple, Ont., estimated his family had acquired about 300 reusable Walmart bags via grocery delivery.

“We can’t return them, we can’t do much with them.”

Now, a little more than a year later, Walmart has launched a pilot project to address the problem.

It allows customers to pack up their unwanted reusable Walmart blue bags and ship them — at no charge — to a facility where they’ll get a second life.

How it works

According to Walmart, bags in good condition will be laundered and donated to charity, primarily Food Banks Canada. Damaged bags will get recycled into other materials. Reusable bags typically can’t go in blue bins because they’re costly and difficult to recycle.

Customers must sign up for Walmart’s program, and enrolment is limited.

Jennifer Barbazza, Walmart’s senior manager of sustainability, said the retailer will fine-tune the details as the program progresses.

“[We] know that some customers have more reusable bags than maybe they need,” she said. “One of the things that we’re really excited to learn about from the pilot is customer acceptance and customer feedback.”

Reusable bags are living rent free in closets and car trunks across the country. Most major retailers made the switch away from single-use plastic bags about a year ago, but it’s taking time for some customers to catch on. They’re forgetting to bring their bags with them, and buying more every week.

Udi Sela has already signed up.

“I definitely think it’s a step in the right direction,” he said in an interview on Friday. “It’s something that needed to be done a while ago. God knows we’ve got a ton of bags kind of piled up.”

He said he’s concerned that some customers may find mailing the bags a hurdle. However, it’s not deterring Sela, who soon plans to ship hundreds. 

Passing the buck?

Not everyone is keen on Walmart’s project. Emily Alfred, a waste campaigner with Toronto Environmental Alliance, said donating the bags to the food bank is just passing on the problem.

“We need to remove waste from the system entirely, and just sending these somewhere else for someone else to deal with is not really a solution,” she said.

Alfred said a better option is a program Walmart piloted in Guelph, Ont., in 2022. For a fee, customers could check out reusable bags from an in-store kiosk and later return them to be cleaned and reused.

“That’s a real circular reuse system,” she said.

Walmart’s Barbazza said the retailer is continuing to explore different reusable bag programs, including ones placed in stores.

She also said she’s confident Canada’s food banks will make good use of the bags.

“There’s definitely a need for sturdy items to distribute materials to the food bank clients.”

The paper problem

Among Canada’s major grocers, only Walmart offers a reusable bag program for all customers.

Loblaw recently switched from reusable to recyclable paper bags for grocery delivery. Sobeys did not respond to requests for comment, but according to its website, the grocer also uses paper bags and “reusable options” for home delivery.

Several environmental experts say paper bags aren’t a good solution, because their production leaves a sizable carbon footprint.

“Paper bags are a problem,” Alfred said. “It takes a lot of energy to recycle paper, takes a lot of trees and energy to make new paper.”

Loblaw said it continues to explore a variety of more sustainable solutions. “It’s a challenge we’re committed to addressing,” spokesperson Dave Bauer said in an email.

Both Walker and Alfred applaud Metro for its grocery delivery program, because the grocer, which operates in Ontario and Quebec, reuses delivery materials.

Metro said customers can get their goods delivered in a cardboard box or reusable bags, which can be returned and used for another delivery. Or customers can opt for a plastic bin and remove their groceries from it upon arrival.

Metro does not offer similar programs for in-store shoppers.

Alfred said the federal government should introduce regulations that mandate retailers adopt effective reusable bag programs for all customers.

“It’s up to our governments and people to demand that these companies do better,” she said.

But Walker suggested that the regulations would be hard to enforce and that incentives could be a better tactic.

For example, if retailers increased the price of reusable bags, shoppers might be less likely to forget them when they head to the store, he said.

“When the cost is a disincentive to do an activity, people change their behaviour.”
